I consent to my dancer having their photo and video taken during rehearsals, for photoshoots and productions which will be used for press coverage, social media and print for the show

I consent for my dancer's photo to be used in the programme that is sold to audience members. I also understand that my dancer's first name will be featured in the programme cast list

The second show will be filmed by a videographer, do you consent for your dancer to be in the video that will be sold to parents of Steren students participating in the show?

All dancers will recieve a show T-shirt, please fill in your dancer's clothing size (in age)
